Key terms, responsibilities and consent requirements for PGx Telehealth.
By purchasing this PGx plus Telehealth bundle, you acknowledge and agree to the following:
Inclusions: This package includes two telehealth consults with a GP provided by MedMate plus the PGx Home Swab Kit provided by Genomic Diagnostics.
Independent clinical care: All telehealth services in this bundle are provided solely by MedMate, an independent third-party healthcare provider. Genomic Diagnostics does not provide medical services, does not control the clinical judgment of MedMate's practitioners, and is not responsible for the care, advice, prescriptions, or decisions made by MedMate or its clinicians.
No guarantee of test referral: The decision to issue a pharmacogenomics test request form rests entirely with the MedMate clinician. Payment for the bundle does not guarantee that a referral will be issued. The clinician will determine clinical appropriateness.
Sharing of clinical information: Genomic Diagnostics will provide the PGx test results to the MedMate clinician who conducted your appointment so that they can interpret the results, discuss them with you, and manage your follow up care. The clinician is responsible for reviewing the results with you and offering any treatment guidance.
Limitations of Genomic Diagnostics' role: Genomic Diagnostics' responsibility is limited to performing the PGx test and delivering the results to MedMate. Genomic Diagnostics is not responsible for any clinical interpretation, advice, diagnosis, treatment decisions or outcomes arising from MedMate's services.
Cancellation: If I cancel my order at any time, applicable fees may apply (for full details refer to the PGx Home Swab Kit Terms and Conditions FAQ).
Personal information: I consent to the transfer of my personal information to MedMate for the purposes of arranging and conducting the telehealth consultations and for Genomic Diagnostics to share the PGx test results with MedMate.
